The Centers for Medicare & Medicaid Services (CMS) has contracted with Yale-New Haven Health Services Corporation/Center for Outcomes Research and Evaluation (YNHHSC/CORE) to study post-discharge outcomes for patients with patients hospitalized with heart attack, heart failure, or pneumonia.

CMS is seeking patients and other interested members of the public to review and to give their opinions about the measures. Patients will be able to share their experiences and give their opinions about the care they received to inform the development of quality measures. CMS is committed to having patient feedback to help better understand population needs.

A report describing measures is posted online. Also, a report with feedback from experts is posted online. These documents are available in the ‘downloads’ section on this website: The project is called “Days in Acute Care after Hospitalization for Heart Failure, Pneumonia, or Acute Myocardial Infarction (AMI).”

If you would like to comment on the measures, please email your comments to small by 5:00pm ET on July 31, 2014. For your feedback to be considered, please submit identifying or contact information. Do not include personal health information in your comments. For more information and to access the above measure documents please visit Please note that at the end of the public comment period, all comments will be posted on the website.

Please contact Faseeha Altaf (at small or 203-764-5700) if you have any questions.
