The Centers for Medicare & Medicaid Services (CMS) has contracted with Yale-New Haven Health Services Corporation/Center for Outcomes Research and Evaluation (YNHHSC/CORE) to study post-discharge outcomes for patients with patients hospitalized with heart attack, heart failure, or pneumonia.
CMS is seeking patients and other interested members of the public to review and to give their opinions about the measures. Patients will be able to share their experiences and give their opinions about the care they received to inform the development of quality measures. CMS is committed to having patient feedback to help better understand population needs.
A report describing measures is posted online. Also, a report with feedback from experts is posted online.
